Unley Park Baptist Church (UPBC) collects the personal information in this form to register and communicate with SALA artists, manage event logistics, and provide relevant updates about the exhibition. UPBC may take photographs during SALA exhibitions for communication and promotional purposes. If you do not wish to be photographed, please let us know when submitting this form.
The action of submitting this completed form electronically takes the place of a signature. This sufficiently satisfies the electronic signature provisions set out in section 10 of the Electronic Transactions Act 1999 (ETA).
